 About Care Partnering

Care Partnering is an independent, privately owned and operated Professional Geriatric Care Management and Alzheimer's Care Consultation practice that specializes in providing expert guidance, support and education to older adults and those who care for and about them.

What does a Professional Geriatric Care Manager (PGMC) do?

  • A PGCM begins by conducting a comprehensive functional and safety assessment of the client's situation.
  • A PGCM develops a personalized plan for care with realistic goals and recommendations.
  • A PGCM identifies, coordinates, implements, monitors and adjusts the care plan as needed.
  • A PGCM liaisons between the client, family, physicians, attorneys, service providers and the community.
  • A PGCM provides ongoing care management.
  • A PGCM provides client advocacy and crisis intervention.

Who is Paulette M. Masse, MHSA, CMC, LSW?

Paulette is well known in Southeastern MA and RI for her passion, expertise and success in working with seniors and their families.  Armed with nearly 30 years of social work experience, an undergraduate degree from Roger Williams University and graduate degree from Salve Regina University, Paulette brings extensive experience and knowledge to her work.  Additional studies include completion of a Certificate program from the Geriatric Health Training Program through Boston University School of Medicine/School of Public Health and the MA Dept. of Public Health and she continues to enhance her skills and knowledge through ongoing educational programs specific to issues related to aging, family and Alzheimer's / Demetia care and research.  Paulette became nationally certified as a Professional Geriatric Care Manager in 2007. 

As an educator, Paulette has spoken at conferences locally, regionally and nationally and has trained both community and facility based caregivers on topics related to aging, Alzheimer's disease and dementia.  Paulette has been a guest speaker at area grammar schools, high schools and colleges as well as community groups and organizations.     

An expert on eldercare issues, Paulette has conducted assessments and prepared reports for the courts in cases where families are in conflict about what is in the best interest of an elder. 

Founder of  the Alzheimer's Partnership of Southeastern MA, Paulette has served on the Board of Directors of the Alzheimer's Association of MA & NH where she Chaired the Patient and Family Care Committee.  Additionally,  she has served as Chair of the Board of Directors at Bristol Elder Services, Inc., on the Community Advisory Committee of the MA Dept. of Mental Health and the Community Advisory Board of Visiting Nurse Services of Newport and Bristol Counties.

Co-founder and facilitator of the first Alzheimer's caregiver support group in the Fall River area (1983) she currently co-facilitates the first early-stage support group in Southeastern MA.  Paulette's dementia care specific experience includes the development and management of the first dementia specific Adult Day Care program in Southeastern MA and a 42 bed dementia specific Special Care Unit in an area skilled nursing facility.
