Professional Geriatric Care Management
- Initial, comprehensive in home/facility assessment to determine needs. This includes but is not limited to assessing a client’s functional and cognitive abilities as well as environmental safety.
- A written report of findings with recommendations.
- Development of a plan for care addressing issues of concern and identifying resources to meet those needs.
- Crisis intervention.
- Service coordination.
- Assisting with a change in living arrangements or relocation if needed.
- Client, family and caregiver support and education.
- Family mediation.
- Expert Witness assessment and report as requested by the court in cases of family conflict.
- Regular visits to a client’s home or residents in the community or a facility.
- Ongoing monitoring of services being provided.
- Ongoing adjustment of the plan of care as the client’s situation changes.
- Advocating on the client’s behalf.
- Liaison with family members, the medical and legal community, service providers, insurance providers, etc.
- Accompanying a client to appointments.
- Accompanying a client to the Emergency Room if needed.
Ongoing care management.
Alzheimer’s / Dementia Care Consulting services include but are not limited to:
- There is no charge for the first call or face to face ½ hr. in which a client or caregiver is requesting information about services or guidance with a particular situation. A caller will be billed after that initial ½ hr.
- Education around such issues as differential diagnosis, disease progression, behavior management, resources, etc.
- Family conflict mediation.
- Any of the services provided under the heading of Professional Geriatric Care Management.
